
Admission Requirements
In addition to completing an application, interested applicants must
submit a current psychological evaluation, a completed 90-L eligibility
form, and a current Individualized Educational Program or equivalent
description of educational services currently being provided by the
client's home parish. Applicants must have an orthopedic impairment
to be considered eligible for admission. This packet of information is presented to the
admissions committee, which includes the facility Medical Director ,
for recommendations on appropriateness of placement. If a client is
approved as a potential resident, admission may take place in as
little as 30 days, during which formal assessment and evaluation is
conducted by the Interdisciplinary Team. After this 30 day period,
the team meets with the client and family to make formal
recommendations regarding placement and programming.
